    Fast physical models for Si LDMOS power transistor characterization

    A new nonlinear, process-oriented, quasi-two-dimensional (Q2D) model is described for microwave laterally diffused MOS (LDMOS) power transistors. A set of one-dimensional energy transport equations are solved across a two-dimensional cross-section in a “current-driven” form. The model accounts for avalanche breakdown and gate conduction, and accurately predicts DC and microwave characteristics at execution speeds sufficiently fast for circuit simulation applications

    Frequency of cannabis and illicit opioid use among people who use drugs and report chronic pain: A longitudinal analysis.

    BACKGROUND:Ecological research suggests that increased access to cannabis may facilitate reductions in opioid use and harms, and medical cannabis patients describe the substitution of opioids with cannabis for pain management. However, there is a lack of research using individual-level data to explore this question. We aimed to investigate the longitudinal association between frequency of cannabis use and illicit opioid use among people who use drugs (PWUD) experiencing chronic pain. METHODS AND FINDINGS:This study included data from people in 2 prospective cohorts of PWUD in Vancouver, Canada, who reported major or persistent pain from June 1, 2014, to December 1, 2017 (n = 1,152). We used descriptive statistics to examine reasons for cannabis use and a multivariable generalized linear mixed-effects model to estimate the relationship between daily (once or more per day) cannabis use and daily illicit opioid use. There were 424 (36.8%) women in the study, and the median age at baseline was 49.3 years (IQR 42.3-54.9). In total, 455 (40%) reported daily illicit opioid use, and 410 (36%) reported daily cannabis use during at least one 6-month follow-up period. The most commonly reported therapeutic reasons for cannabis use were pain (36%), sleep (35%), stress (31%), and nausea (30%). After adjusting for demographic characteristics, substance use, and health-related factors, daily cannabis use was associated with significantly lower odds of daily illicit opioid use (adjusted odds ratio 0.50, 95% CI 0.34-0.74, p < 0.001). Limitations of the study included self-reported measures of substance use and chronic pain, and a lack of data for cannabis preparations, dosages, and modes of administration. CONCLUSIONS:We observed an independent negative association between frequent cannabis use and frequent illicit opioid use among PWUD with chronic pain. These findings provide longitudinal observational evidence that cannabis may serve as an adjunct to or substitute for illicit opioid use among PWUD with chronic pain

    Glycine zinc sulfate penta-hydrate: redetermination at 10 K from time-of-flight neutron Laue diffraction

    Single crystals of glycine zinc sulfate penta-hydrate [systematic name: hexa-aqua-zinc tetra-aquadiglycinezinc bis-(sulfate)], [Zn(H2O)6][Zn(C2H5NO2)2(H2O)4](SO4)2, have been grown by isothermal evaporation from aqueous solution at room temperature and characterized by single-crystal neutron diffraction. The unit cell contains two unique ZnO6 octa-hedra on sites of symmetry -1 and two SO4 tetra-hedra with site symmetry 1; the octa-hedra comprise one [tetra-aqua-diglycine zinc](2+) ion (centred on one Zn atom) and one [hexa-aqua-zinc](2+) ion (centred on the other Zn atom); the glycine zwitterion, NH3(+)CH2COO(-), adopts a monodentate coordination to the first Zn atom. All other atoms sit on general positions of site symmetry 1. Glycine forms centrosymmetric closed cyclic dimers due to N-H⋯O hydrogen bonds between the amine and carboxyl-ate groups of adjacent zwitterions and exhibits torsion angles varying from ideal planarity by no more than 1.2°, the smallest values for any known glycine zwitterion not otherwise constrained by a mirror plane. This work confirms the H-atom locations estimated in three earlier single-crystal X-ray diffraction studies with the addition of independently refined fractional coordinates and Uij parameters, which provide accurate inter-nuclear X-H (X = N, O) bond lengths and consequently a more accurate and precise depiction of the hydrogen-bond framework

    A theory for the impact of a wave breaking onto a permeable barrier with jet generation

    We model a water wave impact onto a porous breakwater. The breakwater surface is modelled as a thin barrier composed of solid matter pierced by channels through which water can flow freely. The water in the wave is modelled as a finite-length volume of inviscid, incompressible fluid in quasi-one-dimensional flow during its impact and flow through a typical hole in the barrier. The fluid volume moves at normal incidence to the barrier. After the initial impact the wave water starts to slow down as it passes through holes in the barrier. Each hole is the source of a free jet along whose length the fluid velocity and width vary in such a way as to conserve volume and momentum at zero pressure. We find there are two types of flow, depending on the porosity, Ăź , of the barrier. If Ăź : 0 = Ăź < 0.5774 then the barrier is a strong impediment to the flow, in that the fluid velocity tends to zero as time tends to infinity. But if Ăź : 0.5774 = Ăź = 1 then the barrier only temporarily holds up the flow, and the decelerating wave water passes through in a finite time. We report results for the velocity and impact pressure due to the incident wave water, and for the evolving shape of the jet, with examples from both types of impact. We account for the impulse on the barrier and the conserved kinetic energy of the flow. Consideration of small Ăź gives insight into the sudden changes in flow and the high pressures that occur when a wave impacts a nearly impermeable seawall

    Partitioning of Co2+ and Mn2+ into meridianiite (MgSO4·11H2O): Ternary solubility diagrams at 270 K; cation site distribution determined by single-crystal time-of-flight neutron diffraction and density functional theory

Available from Elsevier via the DOI in this record.We have grown single crystals of M 2+ SO 4 hydrates at 270 K from aqueous solutions in the ternary systems CoSO 4 –MgSO 4 –H 2 O and MnSO 4 –MgSO 4 –H 2 O. These systems exhibit broad stability fields for a triclinic undecahydrate on the Mg-rich side (i.e., Co- or Mn-bearing meridianiite solid solutions) and stability fields for monoclinic heptahydrates on the Mg-poor side (i.e., Mg-bearing solid solutions of bieberite or mallardite). The solubility curves and distribution coefficients, describing the partitioning of M 2+ ions between liquid and solid phases, have been determined by thermo-gravimetric and spectroscopic techniques. A subset of M 2+ SO 4 ·11H 2 O specimens were selected for single-crystal time-of-flight neutron diffraction analysis in order to evaluate preferential occupancy of symmetry-inequivalent coordination polyhedra in the structure. Considering the nearly identical dimensions of the first coordination shells, there is a surprising difference in the distribution of Co and Mn over the two available sites.This work was supported in part by the Science and Technology Facilities Council, Fellowship number PP/E006515/1 and by STFC grant number ST/K000934/1

    Basking shark breaching behaviour observations West of Shetland

The final version is available from BioMed Central via the DOI in this record.This study reports observations of basking sharks (Cetorhinus maximus) sighted during an offshore geophysical survey conducted in July and August 2013, west of Shetland, UK. During the 38-day survey, trained and dedicated marine wildlife observers recorded 19 sightings of basking sharks (n=22 individuals). Of these observations, 17 were of single sharks, with one observation of two sharks and one observation of three sharks. All surface sightings occurred in water with depths between 129 and 199 m, predominantly prior to noon local time (79%), and were mostly of sharks 6-8 m in length, although a young (2 m) individual was also recorded. Breaching behaviour was observed on 14 occasions, by individuals or in small groups. Breaching has been proposed as a male-male competitive behaviour during courtship displays and female basking sharks may breach to signal their readiness for mating. Aggregations of basking sharks at frontal systems are well documented and linked to the occurrence of prey patches; however, these oceanographic features may also be of importance to courtship. The high number of sightings of sharks recorded during a relatively short time frame in addition to breaching behaviour and presence of young individuals, suggest that this area west of Shetland may be an important habitat for the basking shark

    Experiences and views of patients, carers and healthcare professionals on using modems in domiciliary non-invasive ventilation (NIV): a qualitative study

    BACKGROUND: Advances in technology means that domiciliary non-invasive ventilation (NIV) devices can be remotely monitored via modems in patients' homes. Possible benefits and challenges of modem technology have yet to be established. This study explored the perspectives and experiences of patients, their carers and healthcare professionals (HCPs) on the addition of modem technology in managing home NIV. METHODS: A qualitative study using a combination of focus groups for HCPs and interviews for carers/patients was undertaken. 12 HCPs and 22 patients/carers participated. These focus groups and interviews were audio-recorded, transcribed verbatim and analysed thematically. RESULTS: Five main themes were identified. 'Surveillance: a paradox of findings': HCPs were concerned about unduly scrutinising patients' lives, potentially impacting on HCP patient relationships. Conversely, patients welcomed modem monitoring and did not express concerns regarding invasion of privacy. 'Sanctions': HCPs reported the modem increased access to care and allowed appropriate assessment of ongoing treatment. 'Complacency and ethics': HCPs expressed concerns patients may become complacent in seeking help due to expectations of modem monitoring, as well as being concerned regarding the ethics of modems. There was a suggestion patients and carers' expectations of monitoring were different to that of clinical practice, resulting in complacency in some cases. 'Increased time for patient focused care': HCPs in the focus groups described a number of ways in which using modems was more efficient. 'Confidence: can be improved with technology': patients and carers were positive about the impact of the modems on their health and well-being, particularly their confidence. CONCLUSION: HCPs expressed concerns about surveillance were not corroborated by patients, suggesting acceptability of remote monitoring. Data suggests a need for increased clarity to patients/carers regarding clinical practice relating to responsiveness to modem data. The issue of complacency requires further consideration. Modem technology was acceptable and considered a useful addition by HCPs, patients and carers. TRIAL REGISTRATION NUMBER: NCT03905382

    Periprosthetic acetabular fractures are rare but potentially devastating complications of total hip arthroplasty. As the number of total hip arthroplasties performed annually increases, so has the incidence of periprosthetic fractures, with the topic being spotlighted more frequently in the orthopaedic community. There is a particular sparsity of literature regarding periprosthetic acetabular fractures, with periprosthetic femoral fractures after total hip arthroplasty being traditionally far more commonly reported. This article aims to provide an up-to-date review of the epidemiology, risk factors, diagnostic challenges, classifications, and management strategies for periprosthetic acetabular fractures after total hip arthroplasty
